Introducing GoShare Recall+
Harnessing data to enhance health literacy and chronic disease self-management
Healthily and Pen CS are partnering to create GoShare Recall+, a highly scalable approach to the delivery of consumer education through data-driven capabilities.
The integration of Healthily’s patient education platform GoShare, with CAT4 and Topbar, will enable practitioners to efficiently deliver targeted health promotion campaigns using engaging, evidence-based resources including patient stories, animations and fact sheets sent via SMS.
Pen CS’s CAT Plus – comprising Topbar (decision support tool at point of care), and CAT4 (clinical audit tool for practice population analysis) and PAT CAT (practice aggregation tool for catchment-wide population analysis) – is widely used in primary care around Australia.
How it will work
The Practice Manager or Practice Nurse will extract the data of patients meeting certain criteria (based on local or national health promotion priorities identified by a PHN) using CAT4. A content bundle relevant to the key health promotion priorities will then be prepared and matched to these cohorts of patients. These tailored health resources will then be sent to the groups of patients via SMS.
Example use cases:
- Cancer screening participation: Patients who are about to turn 50 years could be sent information about the National Bowel Cancer Screening Program (patient story, animation on how to use the kit and fact sheet) with the goal of increasing participation.
- Shingles immunisation uptake: Increase uptake of Shingles immunisation amongst the eligible population (people aged between 70-79 years).
- Home Medication Review invitation: Send information on the process and benefits of HMR to eligible patients to encourage them to present for an assessment.
Through CAT4, practices will also be able to report and filter patients who have received a GoShare content bundle. The de-identified patient clinical data, including GoShare activity data, can be sent to a PHN’s PAT CAT. PHNs can then report and filter their population based on GoShare activity – providing an opportunity to evaluate the impact of health promotion activities. This new approach to health promotion represents an efficient way for GP Practices to implement quality improvement measures, increasing participation rates for health initiatives and improving the delivery of health education resources by clinicians.
Due for completion (followed by the GoShare’s integration with Topbar), GoShare Recall+ will disrupt traditional approaches to patient education by providing access to health promotion and services tailored to meet individual and diverse consumer needs.
